As the Care Manager, I am responsible for the daily operations of our service. I find great fulfillment in working closely with both the Coordination and Care teams to provide outstanding care to all our service users. I take pride in managing rosters and care plans to ensure continuity of care for each individual. Additionally, I greatly appreciate the opportunity to conduct initial assessments, which allows me to connect with new service users and customise their care according to their unique needs and preferences.

I find great fulfillment in my role, particularly in providing support to my team and facilitating positive changes. With my expertise in dementia care, I am passionate about working with individuals to overcome the barriers that hinder their acceptance of care. This allows them to embrace the experience and build meaningful relationships with their care team.

I excel at planning our care rounds and schedules, ensuring they are geographically aligned and provide strong continuity of care for our service users. I take pleasure in auditing our care and support plans to ensure they are person-centered and effectively meet the needs of all our service users.

I take pleasure in coordinating care visits, ensuring seamless continuity of care and adequate cover. I appreciate conducting supervisions, as they allow me to support my colleagues when they need assistance. Additionally, I enjoy stepping in for care visits when necessary, as it helps me connect with new service users and stay updated on all service users and their needs and preferences.

I cherish my role because it allows me to deeply understand each service user by learning about their needs, preferences, backgrounds, and interests. Every interaction brings me joy as I discover the unique aspects of their lives. As a care team leader, I strive to be a source of support and reassurance. I take immense pride in being the individual that those I care for, as well as their families, can rely on during life’s overwhelming challenges.
